Insurance Companies
We participate with all major insurance companies except Aetna and United Healthcare . We will gladly see patients with Aetna or United Heathcare under their out of network benefit. In the event that you have an insurance plan that requires you to designate a “primary care provider” (pcp), we ask that you make this change with your insurance company prior to scheduling your appointment. If you need to schedule an appointment prior to receiving a new card with a physician from our practice designated, we ask that you have your insurance company fax confirmation of the change. Without this confirmation, you may be responsible for paying for your visit out of your pocket.
