We are EXCITED that you have chosen our office as your primary care provider. Our goal is to ensure that all of your encounters with our practice be positive. Therefore, we ask that you familiarize yourself with some of our general office policies and insurance information.

UPCOMING CHANGES!
In an effort to better serve your needs, we will be updating our computer system and relocating our billing services to the office effective January 1, 2009. We would like to thank you in advance for your cooperation and patience as we will be asking you to update demographic information and present your insurance card for copying.
